An Ordinance requiring all practicing physicians and midwives to report all deaths, births, that shall occur in their respective practice, to the Health Officer of the City of Ballard, in case there are no attending physician or midwives, said deaths and births shall be reported by the parents of the deceased, of said children, and the Health Officer to report said deaths and births reported to him to the City Clerk.
